Web Banner

Hey guys, I know this is kind oif asking alot but, could someone make a web banner for me? I am not sure what size they are supposed to be. I don't want the ad banner size but like the size at the top of this forum where it says photoshopchop.com. IF anyone wants to do this for me, heres a basic idea that i want. Please confirm with me before you start.

- 3D Grid http://www.thewebmachine.com/
-click on photoshop, then scroll down on the left column and find
"3D grid"
-the words "PC REPAIR" and a phone number i will give you
-an icon/image that I have already made
-colors to be black, silver, blue, and maybe a lil white

If anyone can help, I would appreciate it very much! Thank you!

ya it really is...i just can never get the 3D grid to work...wanna do the banner?
